Before you show up for your first session, please print and fill out the New Patient Information Packet and the New Patient Health Questionnaire. That way we can spend less time filling out paperwork and more time treating you.
Also, depending on where the injury is located, please also fill out an area-specific sheet.
If the patient is under 18 years of age, please fill out the Consent to Treat a Minor.
The day of your session, please arrive 15 minutes early to check-in and complete paperwork.
Please wear comfortable clothing. You will be doing different exercises and we like our patients to be comfortable during their physical therapy session.
