That way we can spend less time filling out paperwork and more time treating you. Also, depending on where the injury is located, please also fill out an area-specific sheet. Contracted Insurance Companies
We are preferred or in-network providers for most insurance companies.
If you do not see your insurance company listed on the right or you do not see "Premier Physical Therapy & Sports Performance", "Tim Kuhn DPT", or "Kuhn Physical Therapy LLC" on your insurance company's preferred provider list, contact us. We are in partnership with Rod Stewart RPT, Ruby Mountain Physical Therapy, and Fallon Physical Therapy so we may be listed under one of these names on your insurance company's preferred providers list.
As a courtesy to you, we call and verify all of your insurance benefits prior to your initial evaluation so there are no surprises.

Q: What can physical therapy do for me?
A: Physical therapy can help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities.
Q: What should I wear to physical therapy?
A: Please wear something comfortable. You will be doing different exercises and we like our patients to be comfortable during their physical therapy session.
Q: What should I expect at my first visit?
A: Plan to arrive 10 to 15 minutes early to your appointment as you will have paperwork to fill out. To save time you can always download and print the forms under our “new patient” section. You will have an examination with your physical therapist and will also spend time going over your symptoms. Depending on your insurance, you may be able to start your physical therapy treatment that day. Your physical therapist will go over all of this with you during your examination.
Q: Do I need a prescription for physical therapy?
A: Depending on your insurance company you may be required to have a prescription for physical therapy. Please check with your insurance prior to scheduling.
Q: Do you file claims with my insurance company?
A: Yes, as a courtesy to our patients we file claims to both primary and secondary insurance companies.
Q: Does my insurance cover physical therapy?
A: As a courtesy we contact your insurance company prior to you coming in for your initial evaluation. Some policies cover a percentage while others require a copay per visit. We encourage our patients to contact their insurance company prior to the appointment with any benefit questions.
Q: How much will it cost if I don’t have insurance?
A: As a courtesy to patients that do not have insurance we offer a cash rate of $100 for your initial evaluation and then $85 for every visit thereafter.
Q: How long is each visit?
A: Each visit will last 60-90 minutes. Time can vary depending on the type of treatment you are receiving.
Q: How many visits will I be seen for?
A: This will be addressed during your initial evaluation; visits vary from person to person.
